The pre-travel consultation of a 69-year-old Cape Town man with metastatic pancreatic cancer with a six-month projected survival time is considered. He has a final wish to perform Hajj in Saudi Arabia, which is 6 weeks away. His fitness to travel, vaccine and medical considerations, religious and social issues present unique challenges. Medical complications during the journey are presented and addressed. At the end of the pilgrimage the airline considers him too ill to be transported back to South Africa and this has to be addressed sympathetically.
